DIRECT PAYMENT AUTHORIZATION EFT FORM - Form is used to furnish bank account identification numbers for once-monthly transfer of assessment payments to the Association. Information is required at least ten (10) days prior to transfer date.
RESIDENT INFORMATION FORM - Form is used to furnish initial or updated personal information which will permit us to make contact with you or your family members when required to service your account.
IMPROVEMENT/VARIANCE REQUEST FORM - Form required to be completed by homeowner and furnished to the Association's Property Value Committee when considering improvements of the exterior of their property. Changes must be approved prior to initiating work on the planned improvements.
SCHEDULE OF LATE FEES, SURCHARGES, AND FINES
CITATION FORM - The Citation Form is used by the Board and managers for citing infractions of Covenants, Bylaws, or Rules & Regulations. The form includes the progression of offenses and the action taken.
SNOW REMOVAL GUIDELINES - Please refer to this guide for an understanding of our snow removal procedures.
MAINTENANCE SERVICES SUMMARY - Please refer to this document for an understanding of the maintenance services which the Association provides. Other than those listed, no services are provided for the maintenance of any residences in the development.
TOWNHOME OWNERS' SPECIAL INSURANCE NOTICE - This form explains the requirement that all owners of townhome units must keep the Association advised of the existence of property and liability insurance coverage on their units. A certificate of insurance must be furnished upon taking occupancy of a unit, and on each subsequent renewal or replacement of the policy.
TOWNHOME CAPITAL FUND DISBURSEMENT REQUEST FORM - According to LPGPOA Covenants, Article II, Section 2.25, Townhome Capital Funds may only be used for the maintenance and repair of the exterior siding, roofs, and driveways of the Townhome buildings. This form must be used by townhome owners when requesting a disbursement from their Townhome Capital Fund.
PROXY TO VOTE - If you are unable to attend the annual meeting and want to vote on matters to be addressed, you must complete this form and return it at least three (3) days prior to the meeting as instructed in the document.
EXPENSE REIMBURSEMENT FORM - For Board and committee members to request reimbursement for expenses for LPG.
FENCE VARIANCE GUIDELINES - The erection of a fence requires the approval of a variance by the Lakepointe Gardens Board of Directors. Please print a copy of the IMPROVEMENT VARIANCE REQUEST FORM from this website, complete it and furnish to the Association, along with a listing of all required materials. The guidelines listed in this form will assist you in determining the restrictions which apply.
PROPERTY LEASING RULES - Our Rules and Regulations Section IX, B states "No lease agreement shall be for a period exceeding one (1) year in length and shall have no renewal options." Section IX explains the procedures required by the owner &/or property manager of any Lakepointe Gardens property which is to be leased or rented to others. LESSOR AFFIDAVIT - Form required to be completed and furnished to the Association by the owner of any leased/rented property within the development. Must be received by the Association before any lease is signed or security deposit is accepted.
LESSEE AFFIDAVIT - Form to be completed by lessee/tenant of any leased/rented property within the development. The property owner should print and furnish the form to the lessee/tenant for completion and endorsement. Must be furnished to the Association before any lease is signed or security deposit is accepted.
